Every image is exported as a PNG file, no matter how it was embedded in the PDF. PNG is lossless and supports transparency, so the extracted images keep their full quality.

Scanned PDFs usually hold each page as a single image. You can extract those full page images, but individual elements inside the scan cannot be separated without OCR.

There is no limit. You can extract every image in your PDF, however many there are. Larger PDFs with many images simply take longer to process.
